I will not disclose who they are (so as not to worry friends and relatives who may read this), but suffice it to say that out of six different households, five have one or more recovering Covid-19 patients.Over 50 percent are now asymptomatic, and me and those who have been spared from the infection to become "remote caregivers" are still keeping a close eye on two more who seem to be long-haulers of this disease. But again, I repeat, everyone is on the road to full recovery. >From the day our first family member tested positive for Covid-19, it has been 19 days of highs and lows. It was on the 13th day ......snip... >... We began to smile again, joke again and laugh again, because of my best friend’s help and a life-saving door she opened for us.I purposely threw all those numbers in my introduction in order to impress upon readers the magnitude of this experience for our family, and every family fighting this virus right now. Imagine, 12 out of 21 family members, whose ages range from 80 years old to 9 years old, testing positive day after day. Children are resilient, yes, but they are not immune from Covid-19. Our Gen Zs and Alphas who got Covid are mostly asymptomatic, but it is nonetheless terrifying to learn of a little niece or nephew testing positive.There is one specific figure though that I want everyone to keep in mind, and that is the number P450. As such, WHO added that it should only be used after clinical trials.As of this writing, there are nine clinical trials for ivermectin around the world. The Philippines is not doing its own trial.Also, the manufacturer of the drug, Merck, said in a statement last February: "We do not believe that the data available support the safety and efficacy of ivermectin beyond the doses and populations indicated in the regulatory agency-approved prescribing information."I have no medical background, but like I said, my best friend may have opened a life-saving door that allowed my family members to consult with an expert Filipino pulmonologist who has reviewed various researches and has therefore based Covid treatment protocols on "real world" studies as opposed to laboratory studies.Not one to insist or force ivermectin on any patient, this very conservative doctor laid out the following points that led my siblings and I (one, in fact, who is very pragmatic and extremely skeptical about the drug) to make the most important decision we have ever made on our family’s health to date. Our pulmonologist has been a frontliner in the fight against Covid-19 and has prescribed ivermectin in combination with other accepted medicines prescribed for Covid-19, and has witnessed its life-saving effects.There are four doctors in our family, while my eldest sister, who has held all our family together during this very difficult time, was a long-time administrator of a government hospital in Metro Manila. They are, you could say, among the most circumspect people during a controversy such as this, but the reasons below are why they are now converts like me.– According to our pulmonologist, ivermectin has been prescribed to humans for the last 40 years, and only 16 deaths have been recorded after over 4 billion doses have been administered as treatment against parasites.– I asked the pulmonologist, "Many say ivermectin works, but to believe it, I would like to ask, in a manner that the general public can understand, how exactly does ivermectin kill Covid-19."Here’s the doctor’s explanation: "Recent studies have shown that ivermectin acts against the virus in seven ways, but I will mention these two which are easy to explain: 1) It hinders the binding of the virus spike protein (remember the spikes of coronavirus, the reason for its name?) with the ACE2 receptors in our cells. (ACE2 receptors are found in lungs, kidneys, heart digestive tract and nose. It is to these cells that the virus will attach); and 2) It disrupts the enzyme (RdRp) of the virus, reducing its ability to multiply. If it cannot multiply, it cannot wreak havoc on our system. The other ways have something to do with the body’s immune response."– The pulmonologist is abreast of ongoing studies at The University of Monash, a public research university in Australia, that has been looking into the "toxic level" of ivermectin for humans. The doctor cited this very study to us even before the drug was released on April 3, confirming that ivermectin kills the virus in 24 to 48 hours.– Ivermectin has been used and is still in use in countries across Africa and Central America where the parasitic disease Onchocerciasis or River Blindness remains a threat. It has long been given at 3 mg per dose, which is why the 12 mg to 15 mg dose of ivermectin for Covid-19 treatment has become controversial today. Nonetheless, these supposed higher doses of ivermectin are way below the dosage for animals and have effectively cured many coronavirus patients.– Take note that the only contraindication that has been documented for ivermectin is if it is given to patients with severe cirrhosis, because "70 to 80 percent of the drug is metabolized or processed by the liver," this doctor said."Moreover, the use of ivermectin on Asians is safe because studies have shown that our livers are ‘fast acetylators,’ compared to Caucasians," he said.Acetylation is a Phase 2 liver detoxification pathway."We are used to giving anti-tuberculosis drugs although these are toxic to some people, and we are not afraid, because, like I said, Asians and Filipinos are fast acetylators. This is why I am confident to give the higher dose of ivermectin," the doctor said.– In this doctor’s experience, ivermectin has been given in the treatment of Covid-19 in four different phases—early, mild, moderate and late stages. More than half of the patients survived.– This doctor also followed real world events in Uttar Pradesh in India, which has a population of 280 million people. There, the government approved the use of ivermectin in August 2020 and started giving it to hospitals and health centers. Since then, their hospital beds have been practically empty.The Manila Times double checked this claim and confirmed it through The India Times and India.com. According to the latter, the recovered cases in Uttar Pradesh are at 601,000, with deaths at 8,881. The state’s total number of cases are around 630,000.Naturally, controversies surrounding ivermectin’s use became part of our consultation with our pulmonologist, which further convinced us how our Covid long-haulers will benefit from the drug. Yet we have within our grasp two long-established drugs with activity against Covid-19 – indomethacin and ivermectin. Both are well-known drugs that have been in regular use by millions worldwide for decades, and have now been widely used in patients with Covid-19.  Early clinical trials have demonstrated their effectiveness for this, and confirm their safety.IndomethacinWe have set out the evidence for indomethacin for treating cough and pain on these pages previously.  Indomethacin use in Covid-19 was first reported in May of this year. Further evidence emerged last week when a preprint of a controlled trial of indomethacin versus paracetamol in hospitalised Covid-19 patients in India was published.  Patients with rt-PCR positive Covid-19 were offered the option of having indomethacin or paracetamol.  A well-matched group of controls was obtained by propensity scoring from amongst those who opted for paracetamol. In the indomethacin group one patient out of 72 required supplementary oxygen compared with 28 out of 72 in paracetamol group; resolution of symptoms (fever, cough and myalgia) was appreciably faster in the indomethacin group. For example, the median time to resolution of cough was 3 days with indomethacin but 8 days with paracetamol.Cough is very distressing, and hard to treat with standard antitussives; it also transmits virus effectively by aerosol when ventilation is poor, as in most homes and older hospitals. Reduced transmissibility alone is now a reason to prefer indomethacin to paracetamol. However, a substantial and growing body of evidence attests to their effectiveness.Neither of these drugs is recommended by the National Institute for Healthcare Clinical Excellence (NICE) – because in order to reach this status there would need to be a randomised, controlled trial (RCT) in the UK to determine the effectiveness of the drugs. This will take many months to approve, organise and analyse even with current infection rates.  The evidence from around the world appears to be disregarded. The search for perfect evidence, such as a large RCT, has become the enemy of the good.A formal proposal was submitted to Covid19 Therapeutics Advisory Panel (CTAP) in October to suggest the inclusion of indomethacin in the RECOVERY trial – but there has so far been no substantive response to this.Many doctors in the UK are reluctant to prescribe drugs "off-label" because of these considerations.  However, they are professionally allowed to prescribe unlicensed drugs if it is required to meet the specific needs of a patient and are "satisfied that there is sufficient evidence or experience of using the medicine to demonstrate its safety and efficacy." In practice, most have followed the prevailing guidance i.e. tell patients to stay at home and take paracetamol or ibuprofen until they become breathless at rest – then call for help from NHS Direct.This cautious approach is at odds with the precautionary principle during a pandemic, and is now throttling the nation’s response to Covid-19 and putting many people at risk of needless suffering.Where does this leave us?We are told that we have growing numbers of cases and face the imminent possibility of hospitals being overwhelmed with risk of doctors rationing treatments as the Lombardy situation is repeated here.It has been argued that there should be four pillars to a pandemic response –1. Contagion control (stop the spread),2. Early home treatment to reduce hospitalisations and death,  3. Late stage hospital treatment and4. Vaccination to achieve herd immunity.Public health authorities (and therefore the media) have focused on three of these but have overlooked early home treatment. Hospital care should be a safety net for survival rather than the first place for treatment.What could we do?We should very quickly review the evidence for indomethacin and ivermectin and establish a protocol for early home treatment of covid-19 to reduce the numbers of late-stage hospitalisations.Indomethacin could be rapidly provided to patients by community pharmacists by means of a patient group direction (PGD) or through an online referral to a prescriber. The mechanisms for these procedures already exist and schemes could be put in place quickly if there were the will to do it.For ivermectin, steps should be taken immediately to authorise its use for Covid-19 prevention and treatment. It could be supplied through the same methods as indomethacin.
